    结直肠癌作为全球第三大恶性肿瘤,在我国的发病率和死亡率持续上升,构成了重大公共卫生挑战。 近年来,早期精准诊疗研究取得突破性进展,为及时干预提供了更多的选择。 在内镜技术创新领域,白光内镜、图像增强内镜等技术,结合人工智能显著提升病变检测的准确性;在新型生物标志物领域,利用外泌体核酸特征的检测系统具有 97. 4% 的灵敏度,以及 AUC 为 0. 981 的多组学整合模型,为非侵入性筛查提供了新的选择;在人工智能应用中,结合组织病理学和内镜特征的机器学习模型已被开发,有助于预测淋巴结转移。 这些多维度进展不仅完善了早期诊断技术,更推动治疗决策向精准化发展。

    Abstract:

    Colorectal cancer, the third most common malignant tumor in the world, has a continuously increasing incidence and mortality rate in our country. That poses a major public health challenge. In recent years, breakthrough progress has been made in early precision diagnosis and treatment research. That provides more options for timely intervention. In the field of endoscopic innovation, techniques such as white light endoscopy ( WLE) and image-enhanced endoscopy ( IEE) enhanced by artificial intelligence (AI), have significantly improved the ability to detect lesions. In the field of novel biomarkers, a detection system utilizing exosome nucleic acid features has a sensitivity of 97. 4% , and a multi-omics integrated model with an AUC of 0. 981. These provide a new option for non-invasive screening. In artificial intelligence applications, machine learning models combining histopathological and endoscopic features have been developed to help predict lymph node metastasis. These multidimensional advancements not only enhance the matrix of early diagnostic technologies but also drive the treatment decision-making toward increased precision.
