    摘要:

    重症监护病房(intensive care unit, ICU)是医院感染(hospital-acquired infectioin, HAI)的高发区,其中最常见的是呼吸道感染,包括医院获得性肺炎(hospital-acquired pneumonia, HAP)及呼吸机相关性肺炎( ventilator-associated pneumonia, VAP)等。医院获得性感染会增加患者负担,恶化临床预后。本文系统分析了医院获得性呼吸感染的分型、流行病学特征、产生机制等,并从八个方面详细阐述了相关防控措施,包括针对耐药菌的筛查、提升手卫生依从性、环境和设备清洁消毒、隔离预防措施的实施、环境微生物监测与管理落实、完善 ICU 室内布局与基础设施及抗生素与多重耐药菌管理等集束化策略。本文旨在为 ICU 内呼吸感染防控提供系统化、科学化的解决方案,以提升医疗质量、改善患者预后。

    Abstract:

    The intensive care unit (ICU) is a high-incidence area for hospital-acquired infections (HAI). The most common of infections are respiratory infections, including hospital-acquired pneumonia ( HAP) and ventilator-associated pneumonia ( VAP). Hospital-acquired infections increase the burden on patients and worsen the clinical prognosis. This article systematically analyzes the classification, epidemiological characteristics, pathogenesis and pathogen spectrum of hospital-acquired respiratory infections. Furthermore, it elaborates on eight key prevention and control measures. The measures include a care bundled management for screening the drug-resistant bacteria, enhancing the hand hygiene compliance, cleaning and disinfecting the environmental and equipment, implementing the isolation precautions, employing the microbial surveillance and environmental management and optimizing the ICU layout and infrastructure as well as improving the management of antibiotics and multidrug-resistant organism ( MDRO). The study aims to provide a systematic and evidence-based solution for ICU respiratory infection prevention. It ultimately improves the patient healthcare quality.
